早期康复锻炼对老年心脏起搏器植入术后心肺功能改善效果

Effect of early rehabilitation exercise in improving cardiopulmonary function after pacemaker implantation in elderly patients

摘要: 目的 探析早期康复锻炼对老年心脏起搏器植入术后心肺功能改善效果。方法 选取2019年1月至2023年6月本院50例老年心脏起搏器植入术患者,随机分为对照组(常规康复护理)与观察组(早期康复锻炼),对比干预效果。结果 两组手术时间对比,差异无统计学意义(P>0.05);观察组比对照组下床正常活动时间、住院时间少,差异有统计学意义(P<0.05)。肩关节活动功能、肩关节功能综合评分方面,观察组术后1周及术后12周比对照组高,差异有统计学意义(P<0.05)。两组术前FEV1/FVC及LVEF对比,差异无统计学意义(P>0.05);术后1周及术后1个月时,观察组比对照组高,差异有统计学意义(P<0.05)。观察组比对照组生活质量评分(一般认知、心理功能、躯体功能、社会功能)高,差异有统计学意义(P<0.05)。并发症发生率方面,观察组是8.00%,对照组是32.00%,观察组更低,差异有统计学意义(P<0.05)。满意度方面,观察组(96.00%)更高,与对照组(72.00%)对比,差异有统计学意义(P<0.05)。结论 早期康复锻炼在老年心脏起搏器植入术后康复护理中应用效果明显,不仅可以改善患者肩关节功能、心肺功能,且有助于其生活质量提高,降低并发症发生率,改善紧张的护患关系,值得采纳、推广。

Abstract: Objective To investigate the effect of early rehabilitation exercise in improving cardiopulmonary function after pacemaker implantation in elderly patients. Methods A total of 50 elderly patients who underwent pacemaker implantation in our hospital from January 2019 to June 2023 were enrolled and randomly divided into control group (conventional rehabilitation nursing) and observation group (early rehabilitation exercise), and the intervention effect was compared between the two groups. Results There was no significant difference in the time of operation between the two groups, and compared with the control group, the observation group had significantly shorter time to ambulation and length of hospital stay (P <0.05). The observation group had significantly higher scores of shoulder joint mobility and function at 1 and 12 weeks after surgery than the control group (P <0.05). There were no significant differences in forced expiratory volume in 1 second/forced vital capacity (FEV1/FVC) and left ventricular ejection fraction (LVEF) between the two groups before surgery (P >0.05), and the observation group had significantly higher FEV1/FVC and LVEF than the control group at 1 week and 1 month after surgery (P <0.05). Compared with the control group, the observation group had significantly higher quality of life scores (general cognition, psychological function, physical function, and social functioning) (P <0.05). The observation group had a significantly lower incidence rate of complications than the control group (8.00% vs 32.00%, P <0.05). The observation group had a significantly higher degree of satisfaction than the control group (96.00% vs 72.00%, P <0.05). Conclusion Early rehabilitation exercise has a marked effect in rehabilitation nursing after pacemaker implantation in elderly patients and can improve their shoulder joint function and cardiopulmonary function, facilitate the improvement in quality of life, reduce the incidence rate of complications, and relieve the intense nurse-patient relationship.
