甲苯磺酸瑞马唑仑、丙泊酚和依托咪酯用于高龄高血压患者无痛胃镜的效果比较

摘要： 目的探讨无痛胃镜中甲苯磺酸瑞马唑仑和丙泊酚、依托咪酯三种不同镇静药物复合舒芬太尼在高龄高血压患者中的效果、不良反应及预后情况。方法选择2020年10月至2021年3月番禺区中心医院行无痛胃镜诊疗患者200例,年龄65-75岁,高血压病史并规律服用药物,随机分为四组（R组、P组、E组和S组）,每组50例。比较四组患者麻醉前（T0）、开始检查后1min（T1）、睁眼后（T2）的MAP、HR、SPO₂。统计四组不良反应发生率情况,同时对四组术后苏醒时间及离院时间进行比较。结果 R组患者平均动脉压、心率、血氧饱和度在T1、T2点显著高于P组。与P组、E组比较,R组苏醒时间、离院时间明显缩短（P<0.05）。与R组比较,P组注射疼、低血压、低血氧发生率增高,E组术中肌颤发生率增高,S组体动、呛咳、术后恶心呕吐发生率增高（P<0.05）。结论甲苯磺酸瑞马唑仑对高龄高血压患者循环、呼吸影响较小,不良反应发生率低,苏醒及离院时间缩短,更适合应用于高龄高血压无痛胃镜检查中。
