增强型体外反搏联合心脏康复训练治疗慢性心力衰竭的随机对照研究

摘要: 目的 探讨增强型体外反搏(EECP)联合心脏康复(CR)训练治疗慢性心力衰竭(CHF)的临床疗效。方法 选取2020年6月至2021年6月期间在南通市通州区人民医院康复科接收心脏康复治疗的慢性心力衰竭患者共164例为研究对象。随机分为对照组和实验组各82例。对照组给予传统的心脏康复(CR)训练;实验组则在对照组的基础上给以增强型体外反搏(EECP)治疗。结果 (1)治疗后两组患者的LVEF高于治疗前、SV低于治疗前,比较均有统计学意义(P<0.05),且治疗后实验组的LVEF更高、SV更低,组间比较有统计学意义(P<0.05)。(2)治疗后两组患者的FVC高于治疗前,而FEV1指标低于治疗前(P均<0.05),治疗后两组患者两组指标组间比较差异均无统计学意义(P均>0.05)。(3)治疗后两组患者的Peak VO2、AT均高于治疗前(P均<0.05);治疗后实验组的两组指标均高于对照组(P均<0.05)。结论 增强型体外反搏联合心脏康复训练在改善慢性心力衰竭患者心功能、运动耐力方面效果要优于单纯的心脏康复训练。

Abstract: Objective To investigate the clinical effect of enhanced external counterpulsation (EECP) combined with cardiac rehabilitation (CR) training in the treatment of chronic heart failure (CHF). Methods A total of 164 patients with CHF who were hospitalized and underwent CR training in Department of Rehabilitation Medicine, Tongzhou People’s Hospital, from June 2020 to June 2021 were enrolled as subjects, and they were randomly divided into control group and experimental group, with 82 patients in each group. The patients in the control group received conventional CR training, and those in the experimental group received EECP in addition to the treatment in the control group. Results After treatment, both groups had a significant increase in left ventricular ejection fraction (LVEF) and a significant reduction in stroke volume (SV) (all P<0.05), and the experimental group had a significantly higher LVEF and a significantly lower SV compared with the control group (both P <0.05). After treatment, both groups had a significant increase in forced vital capacity and a significant reduction in forced expiratory volume in 1 second (all P <0.05), and there were no significant differences in these indicators between the two groups after treatment (both P >0.05). After treatment, both groups had significant increases in peak oxygen uptake (PeakVO2) and anaerobic threshold (AT) (all P <0.05), and the experimental group had significantly higher PeakVO2 and AT than the control group (both P <0.05). Conclusion EECP combined with CR training has a better clinical effect than CR training alone in improving the cardiac function and exercise tolerance of patients with CHF.
