Objective
To investigate the application value of nursing intervention with chronic disease trajectory mode combined with incentive nursing in elderly patients with coronary heart disease.
Methods
Based on strict inclusion and exclusion criteria, 76 elderly patients with coronary heart disease who were admitted from July 2018 to March 2022 were enrolled and divided into control group and observation group according to the principle of systematic sampling, with 38 patients in each group. The patients in the control group were given routine nursing, and those in the observation group were given nursing intervention with chronic disease trajectory mode combined with incentive nursing. The two groups were compared in terms of psychological state, the incidence rate of adverse cardiovascular events, and quality of life.
Results
Before intervention, there were no significant differences in Self-rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) scores between the two groups (

>0.05), while after intervention, the observation group had significantly lower SAS and SDS scores than the control group (

<0.05). The observation group had a significantly lower incidence rate of adverse cardiovascular events than the control group (

<0.05). After intervention, compared with the control group, the observation group had a significantly higher quality of life score based on 36-Item Short-Form Health Survey (

<0.05).
Conclusion
In elderly patients with coronary heart disease, nursing intervention with chronic disease trajectory mode combined with incentive nursing can alleviate anxiety and depression, effectively control the occurrence of adverse cardiovascular events, and comprehensively improve the quality of life of patients, thereby achieving a good prognosis.
